Lisa McClammy, BSN, RN, COS-C, HCS-D, HCS-O
Title: Vice President Quality and Education, McBee
Lisa McClammy has more than 24 years of nursing experience in various areas of healthcare. She currently holds certifications in OASIS-D, and ICD 10 coding. Lisa is a Senior Clinical Education Consultant for MAC Legacy. She has presented for various state associations and well as conducting training and education on a variety of topics. Her experience as a consultant, Director of Customer Success for a Home Health Software company, and Director of Patient Care and Alternate Administrator for a home health agency give her a unique perspective and an understanding of the challenges home care agencies face. Lisa is an active member of the Texas Association of Home Care & Hospice for the last 6 years, serving on both the Medicare/Medicaid and Clinical Practice committees.
Rachel Hammon, BSN, RN
Title: Executive Director, TAHC&H
Rachel Hammon received her Bachelors of Science degree in Nursing from the University of Texas in Austin. She has 29 years of experience in nursing with 25 years’ experience in the home health industry. During her employment with a home health agency, she served in many roles (i.e. Pediatric Case Manager, Medicare Case Manager, DON / Administrator and Corporate Director of Clinical Services), which gave her a diverse knowledge of the industry. At the Texas Association for Home Care and Hospice Rachel assisted member agencies with compliance with state and federal laws (Medicare, Medicaid, OSHA, BON, etc.), served as a liaison with state and federal government officials, and on the Nurse Practice Advisory Committee for the Board of Nurse Examiners. As the Executive Director, Rachel is responsible for implementing the mission of the association which is to advocate for ethical practices, quality, and economic viability of licensed providers in Texas. As part of that advocacy she routinely works with state and federal legislators and agency staff on legislation and policy that affects the home care industry.
Colleen Bryan, MSNED, RN
Title: Senior Consultant, Primary Nurse Planner, JCC Consulting
Colleen Bryan has worked in the home care industry since 1996, holding various positions in both home health & hospice agencies including field nurse, case manager, educator, QA manager, DON and Administrator. Colleen is currently a senior consultant and lead educator with JCC Consulting, and services home health and hospice agencies in Texas.
Colleen enjoys helping agency leaders navigate the constant changes and challenges in the current home health and hospice industries. She enjoys assisting in the incorporation of meaningful and impactful changes into their agency’s operations – both administrative and clinical, in order to provide high quality care that is both sustainable while maintaining regulatory requirements and promoting excellent patient care.
Jennifer Elder
Title: Director of Regulatory Affiars
Jennifer Elder started with TAHC&H in September 2019. Her experience includes Home Health and Hospice Administration, DME Management and Home Care Consulting. Prior to coming to TAHC&H she worked as a consultant from 2012 – 2017 for home health and hospice agencies and DME agencies throughout Texas. She has hands on experience in the home care industry with the ADR process, QAPI programs, Marketing and Business Development, Licensing, Accreditation, Plans of Correction and Informal Dispute Resolutions, Human Resources, Policy Development and Strategic Planning. Jennifer has been in the home care industry since 2007.
Katie Wehri
Title: Director of Home Health & Hospice Regulatory Affairs, National Association for Home Care & Hospice
Katie has 25+ years in the hospice, home health, private duty and palliative care industries holding various executive level positions in these arenas. She has extensive experience in operations, healthcare regulation and accreditation standards interpretation and compliance, quality assessment and performance improvement programming and compliance programs. She has worked for hospices and home health agencies in a variety of settings including multiple locations in multiple states, hospice inpatient unit, pediatric hospice and adult and pediatric palliative care. She also has experience in opening and expanding sites for hospice and home health organizations. Katie is educated in health care administration with a bachelor’s degree from Bowling Green State University in Ohio. She is currently the Director, Home Care & Hospice Regulatory Affairs for NAHC.
J’non Griffin, RN MHA HCS-D, COS-C, HCS-H
Title: Sr. Vice President/Principal-Coding and OASIS Department, SimiTree
J’non Griffin is a 34 year veteran of home care as an RN. She received her Master’s Degree in Health Care Administration in 2005. She has experience as a field nurse, director and executive with home health and hospice agencies, both large and small. J’non is certified as a Homecare Coding Specialist-Diagnosis (HCS-D), Certified in OASIS competency, COS-C, Certified in Homecare Coding Specialist-Hospice (HCS-H), Home Care Specialist-Compliance (HCS-C) and is an AHIMA approved ICD-10-CM trainer/ambassador. J’non is an accredited ACHC and CHAP consultant. She has presented several webinars, and has been a nationally recognized speaker on a variety of subjects.
Melinda Gaboury, COS-C
Title: Chief Executive Officer, Healthcare Provider Solutions

Melinda A. Gaboury, with more than 28 years in home care, has over 18 years of executive speaking and educating experience, including extensive day to day interaction with home care and hospice professionals. She routinely conducts Home Care and Hospice Reimbursement Workshops and speaks at state association meetings throughout the country. Melinda has profound experience in Medicare PDGM training, billing, collections, case-mix calculations, chart reviews and due diligence. UPIC, RA, ADR & TPE appeals with all Medicare MACs have become the forefront of Melinda’s current impact on the industry. She is currently serving on the NAHC/HHFMA Advisory Board and Work Group and is Associate Director on the Home Care Association of Florida Board of Directors. Melinda is also the author of the Home Health OASIS Guide to OASIS-D1.
